To obtain the reduced sterilization surgery price ($40 off the regular (office) surgery price for dogs), you must meet one of the following:

1. 1.Be a member of a registered rescue organization bringing a rescue dog or cat that is up for adoption
   

2. 2.Have proof of government assistance (must bring photo id)
   

        Section 8 housing voucher

        Schedule letter from Ohio Works First

        Medicaid card

        Letter from Veterans Benefit Administration

        Ohio Direction Card (food stamps)

        WIC card

        Supplemental Security Income

        Social Security Disability

3. 3.Tax return showing that the family income does not exceed 150% of the federal poverty guidelines
   

4. 4.Currently enrolled in college (current tuition receipt required)
   

5. 5.Bringing 3 or more dogs for sterilization on the same day (same household only)
   

6. 6.Proof that the pet has been adopted from a non-profit humane organization
